W-3 YES Width of Box b expanded to accommodate 5 new check boxes, and all other information has been pushed down 2 lines.  Box 9 (Advance EIC payments) and Box 12b (HIRE exempt wages) boxes have been grayed out.   Boxes 16 and 17 have moved to the left side of the form
1042S YES Width of Boxes 3, 4, 5, and 6 changed. Height of Boxes 5 and 6 reduced to 1.5 lines (???).  Boxes 4, 5, and 6 moved. 
1096 YES Two new check boxes added in such a way that reprogramming will be required for every type of filing except W-2G.
1098-C YES "Corrected" check box and 4a check box moved left. Boxes 6a, 6c, and 7 check boxes moved right apparently just to annoy filers.
1099-A MAYBE Removed one check box and moved the other..
1099-B YES Boxes 3, 5, and 6 wording changed. check box added to Box 6.  Former Box 3 is now new Box 14. Old box 7 split into Boxes 7 and 8. Old boxes 8 and 9 now combined in new Box 9.  Old Box 12 has been split into new Boxes 12 and 13.  Two new Boxes, 14 and 15 have been added.  Account Number and CUSIP number box positions have been switched to maximize confusion.

 		Removed one check box and moved the other. Box 5 label changed.
1099-DIV YES Changed size and location of Boxes 2a, 2b, 2c, 2d.  "2nd TIN not." check box and "Corrected" check box moved.
1099-H MAYBE No changes to form layout, but a new "Copy 1 for Recipient' has been added.
1099-R MAYBE New Box 10 "Amount allocable to IRR within 5 years" added beside Box 11.

 Boxes 12a, 12b, 13a, 13b, 13c are shifted up and re-sized.
